Certificants Update Form

The QEEGBoard needs to maintain a full slate of information about every certificate holder, more than what is displayed online. The online display is simply a subset so your credentials can be recognized and those who want to utilize your services have a way to find you yet not provide so much data that marketers can pester you. The fields in yellow are ones that can be seen online. If you do NOT want certain fields seen online, you may check the nearby box "Do Not Publish."  Please supply all data elements to QEEGBoard.

Even if you are just making some changes, please type your data into ALL the fields carefully and then proof your entries.
